Welcome to Acuity Mental Health! 





Please read the information below before scheduling your initial appointment.
  • Your first appointment is a diagnostic assessment (up to 60 minutes). You and your therapist will discuss your current symptoms, as well as your history and treatment needs. Your therapist will determine an initial diagnosis (required for insurance processing) and answer any questions you have about the treatment process.
  • Once your initial appointment is confirmed by your therapist, you you will receive an email directing you to the client portal in order to complete the required forms. Please be sure to review and sign these forms prior to your your first appointment
  • You are responsible for your copay (if using insurance) or the agreed upon fee (if not using insurance) at the time of your initial appointment.
  • If you plan to use insurance to cover the cost of your sessions, please be sure to bring your insurance card with you to your first appointment (it is also recommended that you call the number on the back of your insurance card to confirm coverage).
  • IMPORTANT: If you need to cancel or reschedule your initial assessment you must contact Charmagne via TEXT or EMAIL at least 2 days (48 hours) prior to the scheduled time
  • Appointments not cancelled prior to 48 hours will result in a $50 late cancel fee.
